* Financial Aid is available if you do not have insurance and you meet the income levels in the sliding fee scale. View the sliding fee scale here. Just mention your need to the office staff when you are scheduling your appointment. In order to receive financial aid, please bring along the first two pages of your Income Tax Return and the most recent pay stubs of everyone working in your household. If you need special provisions beyond the fee scale go to: fill out the counseling financial aid application.
